Some of the products I want shipped from a particular zip code (with other products in different categories) are in sub-categories. How does this affect how the "packing by groups" feature?

The module is not capable of recognizing that a category is a "sub-category". The only category level configuration that it recognizes is the one to which the products are assigned. If a products is assigned to more than one category, it will look through each category to which the product is assigned until it finds one that is configured and it will use that category's configuration.
